Application Forms for admission into the Community Health Officers’ Training School, University of Uyo Teaching Hospital for the 2021/2022 School Session is now on sale.

Admission Requirements

• Applicants must be holders of Diploma in Community Health with not less than 2 years post qualification experience or Community Health Extension Workers Certificate with 2 years experience.
• All Applicants must have 5 credits level passes in SSCE, WASCE, NECO or GCE OIL at not more than 2 sittings which must include English Language, Mathematics, Biology, Chemistry and Physics.
• In addition, all Applicants must be registered with the Community Health Practitioners Registration Board of Nigeria and possess a valid practicing license.

Method of Application

Application forms are available at the Community Health Officers’ Training School, University of Uyo Teaching Hospital, Uyo at the cost of & N10,000.00 (Ten Thousand Naira) only. Closing date is 11th June, 2021. 

Entrance Examination Date

Entrance examinations dates will be communicated to the Applicants after the closing date.
