FUHSA Azare UTME/DE Admission Physical Screening 2023/2024:Federal University of Health Sciences Azare wishes to invite the 2023 UTME/DE admission applicants for Physical Screening Exercise scheduled to hold as follows:
Wednesday: 30th August, 2023
- Medicine & Surgery (MBBS) and
Dentistry (BDS) Candidates
Thursday: 31st August, 2023
- Nursing Candidates
Friday: 1st September, 2023
- Radiography/Nutrition & Dietetic
Candidates
Time: 9:00am – 4:00pm Daily
Venue: University ICT Centre, Azare

Candidates must bring originals of all their documents and one set of photocopies. The documents expected are:
- UTME Result slip
- Acknowledgement Form (printed from the FUHSA Post-UTME Portal)
- ‘O’ Level Result (WAEC/NECO)
- ‘A’ Level Result/ First Degree certificate (for Direct Entry only)
- Scratch cards for ‘0’ level result(s) submitted
- Evidence of Local Government Area of Origin
- Birth Certificate or Sworn Affidavit of Age Declaration
