1. National Diploma in School of Health Information Management

Application and eligibility

Applications are hereby invited from suitably qualified candidates for admission into the National Diploma (ND) programme, (full-time) of the School of Health Information Management for the 2020/2021 session.

Applicants must be candidates who sat for the Joint Admissions and Matriculation Board Unified Tertiary Matriculation Examination (UTME) Conducted in 2020.  The candidates must have a minimum score of 120 to be qualified to apply and to be screened.

 Admission Requirement/Educational qualification

  • In addition to a minimum score 120 in 2020 UTME which requires any three subjects from Mathematics, Biology, Physics, Chemistry, Geography,  Economics, Yoruba, Government and Religious Knowledge
  • Candidates should also posses the West African School Certificate/General Certificate of Education/SSCE/NABTEB/NECO with 5 passes at credit level in five (5) papers obtained at not more than two (2) sittings
  • These papers must include English Language, Mathematics, Biology/Health Sciences, Chemistry/Physics and any other 1 subject from Economics, Government, Geography, Religious Knowledge and any Nigerian language.

Duration of Course

  • Two (2) calendar years commencing from October, 2020 (Full-Time Programme).

 

Accommodation

  • The course is non-residential. However, there is limited accommodation on first come first serve basis.

  • 31st   August, 2020.

Applications are hereby invited from suitably qualified candidates for admission into the Higher National Diploma (HND) programme, (full-time) of the School of Health Information Management for the 2020/2021 session.

  1. Admission Requirement/Educational qualification for Higher National Diploma Programme.
  • Applicant must meet the requirement for National Diploma Admission. He/She must possess National Diploma Certificate or Health Records Technician Certificate.
  • Candidates with ND certificates in Health Information Management must have evidence of one year industrial training.

3. ENTRY REQUIREMENT INTO THE POST-GRADUATE SCHOOL OF HEALTH INFORMATION MANAGEMENT (PGSHIM).

  1. PROGRAM

The name of the program is Post-Graduate Diploma in Health Information Management (PGDHIM). Affiliation to University of Ilorin in progress.

  1. ENTRY REQUIREMENT

Candidates seeking admission into Post-Graduate School of Health Information Management shall normally possess at least five (5) credits pass in O’level certificate in not more than two sittings in Mathematics, English language, Biology, Chemistry and Physics, HND or Degree in Health Information Management from a recognized institution.

  1. AFFILIATION

Affiliation to University of Ilorin (in progress).

  1. CLOSURE OF SUBMISSION OF APPLICATION FORMS

The Application forms are to be filled from the University of Ilorin Teaching Hospital website (www.uithilorin.org.ng) pay Ten Thousand Naira (N10, 000.00) non-refundable fee into University of Ilorin Teaching Hospital Account with the CBN through CBN e-payment gateway REMITA.

  1. All the data and scanned uploads (e.g Certificates, Remita reciept e.t.c) on the form fields MUST be supplied correctly as required for your form to be processed.
  2. Payment MUST be made first before proceeding to the form page
  3. To make payment Log on to www.remita.net
  4. Click on Pay FGN and State TSA
  5. Select and click FGN Federal Government of Nigeria
  6. Type University of Ilorin Teaching Hospital on whom do you want to pay
  7. Select UITH GENERAL PURPOSE on Name of Service/Purpose
  8. Fill in the necessary information and make your payment.

  10. After payment is made through the Remita Platform, you can now proceed with your registration by filling the online SHIM FORM with the Remita Retrieval Reference (RRR) Number generated with the payment made.
  11. Click APPLY NOW below, then it will redirect to the interface where you will be asked to enter a VALID personal E-mail address.
  12. A confirmation link will be sent to the E-mail you provided during the E-mail registration.
  13. Login into your E-mail account by opening a new Tab in the same browser already opened.
  14. Click on the link sent to your E-mail by noreply@uithilorin.
  15. The SHIM application form will be made available for you to fill and submit.
  16. Select the appropriate programme you wish to apply for in the drop down menu.
  17. Fill every other field on the form as required.
  18. After submission wait for 2 seconds as you will be directed to print or download the filled form.
  19. NOTE: Please don’t bring the printout to UITH.
